Initiating Membership
Memberships must be initiated in-person or by phone. Plans are subject to a one-time initiation fee of $200.
3 month minimum Commitment required.
Direct primary care (DPC) memberships include unlimited primary care visits. Membership enrollment includes the new patient intake, at no extra cost.

*The new patient intake consists of comprehensive 90 min intake, followed by a 45 min appointment to review lab results & discuss treatment plan. Both visits are included in the price above.
